At Abode Care Partners, our physicians and nurse practitioners (NPs) specialize in providing care to seniors across the continuum of care. Our model of care emphasizes prevention, early interventions, monitoring medication changes, addressing emergent issues, family education, timely order completion, and more, to improve the quality of life for residents and increase the length of stay.
Our primary care dedicated NPs deliver holistic, personalized care that helps keep patients healthy and active at all life stages.
Our model of care allows us to deliver a portion of value-based care shared savings to each Senior Living Community. Additionally, we will provide you with a customized estimate of your shared savings value.
Our compassionate primary care teams, including physicians and nurse practitioners, treat residents like family.
Our proactive care model emphasizes prevention and early intervention which helps residents avoid hospitalizations and emergency room visits, keeping them engaged with their day-to-day activities.

- Physician and Nurse Practitioner Visits/Annual Wellness Visits: We prioritize convenience and comfortability with visits wherever patients call home.
- 24/7/365 Care Coverage: Nurses drive improved outcomes by being available to triage and answer resident and family questions any time.
- Chronic Disease Management: NPs assess and treat chronic diseases such as diabetes and hypertension, maximizing quality of life.
- Palliative Care: In every care plan, we focus on resident and family goals and prioritize comfort.
- Educational Resources: NPs deliver support and education for partnering staff, residents, and families through direct conversation, virtual sessions, and caregiver learning events.
- Coordination of Care: We offer this service to all of our patients.
- Language Translation Services: Available upon request.
- Medication Optimization: Clinicians work directly with each community’s pharmacy consultant and community leaders to collaboratively reconcile medications and deprescribe, as needed.
- Care Management Support: Delivering patient-centered transitional care from hospital to home or community settings.
- Cognitive Support: Therapy ordered to assess and maximize cognitive abilities.
- Wound Care: Weekly wound rounding and on-site debridement/grafting.
- Fall Prevention Assistance: Assessing the home or residence for fall risks and offer prevention tips and recommendations.
- Remote Patient Monitoring: Tracks most current vital signs preventing acute issues and transfers, allowing families and community staff to be up to date on health status.
- On-site Clinics: On-site Clinics for patients, families, and employees.
